Position Title:
L.S.W. (part time)

Position Overview:
Responsibilities include, but are not limited to: documentation, annual reports, assessments, quarterly reports, intake assessments and reports, hospitalizations (discharge planning), attend meetings (mandatory), discharge planning (consumer exits from program), admission assessments, social history.

Requirements:
Applicant must be a licensed social worker & possess certification of insurance from the state of Pennsylvania. Position would require 25 hours per month and/or 300 hours per year for 3 facilities (23 consumers). Familiarity with ICF/MR regulations preferred.
