Mental Health Coordinator Registered Nurse is needed to provide cross program care management support, consultation, risk evaluation/reduction and other supportive services associated with elderly consumers who exhibit complex problems that may be related to mental health diagnosis.  This position will be involved in developing more effective treatment and support plans for consumers in a consulting capacity.  RN will identify systemic trends that indicate a need to develop additional support services and training and then participate as a key part of the development of resources to meet the identified needs.  Additional responsibilities will include:

  • Reviewing medical, behavioral and support system issues to develop a holistic and strength based case management approach for consumers
  • Meeting with care managers, caregivers and others involved in the support of consumers to discuss services and to develop a more effective system of service delivery
  • Monitoring consumers services to determine the effectiveness and quality of services
  • Making home visits as needed, consulting with consumer, program staff and providers regarding consumer needs, care planning, consumer status and services provisions
  • Developing and maintaining relationships with service providers and collaborating with them to assure best service delivery
  • Recommending procedures and guidelines for clinical aspects of the program.
  • Representing and advocating for the best clinical practice, working to integrate medical and social aspects of intervention resources and strategies
  • Serving as an intermediary between physicians, care managers and human service system
  • Assisting AAA care management with contacting physicians or accompanying care manager on consumer visits
  • Acting as a medical advisor to the Area Agency on Aging (AAA) and AAA providers

Registered Nurse with an active license, plus 3-5 years of mental health experience. Experience in long-term care/geriatrics preferred, but not required.  Highly organized individual, with excellent communication skills and the ability to work independently is important.  Position involves travel, so a valid driver’s license and access to a reliable vehicle are required.
