Adult Case Manager, East End Cooperative Ministry - Pittsburgh, PA

Reports to:       Emergency Homeless Services Manager

This position exists to provide case management services to assigned clients to assist them in reaching their goals.  This full-time position operates on a five-day work week, including working two days from 1pm to 9pm.

Duties

  1. Provide case management services to the clients at the Men’s Shelter, the Drop-In, Orr Center, Safe Haven and assigned community locations.
  2. Perform strength-based assessments with assigned clients.
  3. Develop Services Plans built on identified strengths to help the clients reach their goals.
  4. Develop and maintain working relationships with referral agencies so that our clients will be accepted.
  5. Monitor the quality and effectiveness of those services provided by others.
  6. Provide individual and group based chemical dependency services to assigned clients.
  7. Document all services provided in client records.
  8. Perform admissions into the program.
  9. Assist with the acceptance, maintenance and distribution of donated items.
  10. Assist with planning and scheduling of appropriate activities and field trips.
  11. Participate in the Leadership Team and other meetings to plan for the optimal functioning of the program.
  12. Participate in scheduled staff meetings.
  13. Participate in the mandatory training and staff development training, as requested.
  14. Provide limited on-call services if both the Supervisor and Manager are scheduled off, in keeping with established procedures.

Qualifications

  1. Bachelors degree or three years of experience in one or more of the following:  geriatrics, mental health of substance abuse; pastoral care; social work; or the equivalent in training and experience, as approved.
  2. Must have good communications skills including writing skills.
  3. Must be able to communicate verbally and telephonically.
  4. Must be able to effectively observe and hear situations.
  5. Must be able to quickly and safely climb stairs in the course of everyday employment as well as in response to an emergency and to provide assistance to guests.
  6. Must be able to quickly and safety exit the building and to provide assistance to guests.
  7. Must be computer proficient with Microsoft Office products as well as database experience, email, etc.
  8. Act 33 and 34 clearances.
  9. Must possess a reliable vehicle as well as a valid PA driver license with a three (3) year continuous clean driving record.
