JOB DESCRIPTION
JOB TITLE: Patient Services Coordinator
REPORTS TO: Director of Patient Services
FLSA STATUS: Exempt, Full-Time
Summary: The Patient Services Coordinator will provide ALS patients, families and caregivers with information about ALS, community resources, social services and disease management. Further, the PSC will coordinate Chapter Programs & Services, including, but not limited to respite care, transportation, equipment loan, resource/support groups and educational symposia.
Qualifications:
- BSW/Medical Social Worker, RN, or LPN; other healthcare professionals may be considered
- Ability to assess both physical and psychosocial needs of ALS patients and families, based on knowledge of ALS, the disease and its ramifications
- Possess knowledge of community resources/social services.
- Excellent communication skills, both written and verbal.
- Ability to use computer effectively including specifically MS Word, MS Excel, email proficiency.
- Must have a valid Driver’s License and reliable transportation to meet service area demands
- Willing to travel to outlying areas to meet with patients, families and health care providers.

Responsibilities:
- Responds to patient and family inquiries for information and referral including all referrals received from the National ALS Association Office.
- Provides telephone consultation on a regular basis with set day(s) and time period.
- Provides home visits to patients on a limited basis or by specific requests.
- Coordinates and arranges for Chapter Patient Services to provide supportive services to patients, families, and caregivers, such as transportation and equipment needs.
- Provides referrals to community resources, social service agencies, care providers, and government services.
- Maintains patient and family records for Chapter files and maintains accurate documentation and case notes in DonorPro and patient files.
- Offers professional opinions or recommendations to Patient Services Director and/or Chapter Patient Services Committee concerning current or future Patient Services Programs.
- Assists with planning and facilitating monthly support group meeting (if not group facilitator or if more than one support group, works closely with support group facilitators in meeting planning).
- Contributes or reviews articles pertaining to patient care and information for the chapter newsletter.
- Maintains updated information on ALS, ALS specific research, patient care techniques, and equipment.
- Represents the Chapter at an affiliated ALS Association Center or ALS Clinic if required.
- Participates in continuing education seminars, workshops and conferences as available and necessary.
- Additional duties and activities as required.
